This policy describes what we collect on connecthealthnow.com, how we use it, and the choices you have. We are not an insurance company, agency, or licensed agent, and we are not affiliated with any government health program.

Information you submit: name, phone number, email address, state, ZIP code, and household coverage details you provide in our form, along with a record of the consent language you agreed to, the time of submission, and the page you submitted from.
Automatic information: IP address, device and browser details, and site usage data collected via cookies and similar technologies, including advertising pixels (such as the Meta pixel) used to measure and improve our advertising.

How we use and share your information — including sale/sharing

We collect your information for one primary purpose: to connect you with independent licensed insurance agents and agencies ("partner agents") who can discuss health coverage options with you. When you submit our form with the consent box checked:

We sell or transfer your submitted information to one or more partner agents, who may contact you by phone (including autodialed and prerecorded/artificial-voice calls), SMS text message, and email. Our current partner agents are listed on our Partner List page, which is updated as partners join or leave.
We and our service providers (hosting, lead management, analytics, communications) process your information to operate this site, route your request, prevent fraud, and comply with law.
We do not share your form information with advertising platforms; advertising pixels receive only event and device data described above.

Your rights — Do Not Sell or Share, access, deletion
